The questionnaire is completed with a comprehensive report
Veterans Affairs Canada may request the Psychiatric/Psychological Condition(s) Medical Questionnaire, PEN 6248, as part of a disability benefit application, reassessment, rehabilitation process, or another authorized review. The questionnaire helps VAC understand the diagnosis, current symptoms, treatment needs, and functional impact of the condition.
As a standard part of this service, Dr. Marcinkevics completes both the VAC medical questionnaire and a comprehensive psychological assessment report. The report provides a fuller account of the assessment, including diagnostic findings, symptom history, functional impact, the relationship of the condition to service when requested, prognosis, and treatment recommendations.
The report is written to answer the VAC referral clearly and to remain useful to the client as a detailed summary of the findings, treatment recommendations, and suggested next steps.
The role of the psychologist is to provide an independent clinical assessment and answer the requested medical questions. The assessment does not determine entitlement and cannot guarantee a particular VAC decision.

Relevant medical, psychological, military, RCMP, and treatment records are considered when available and authorized.

Both the VAC medical questionnaire and a detailed psychological report are completed as standard assessment deliverables. The report may also be provided directly to the client.

Both documents are completed and returned through the method authorized for the file. Subject to the referral requirements and informed consent, the comprehensive report can also be provided directly to the client as a practical reference for understanding the assessment findings, treatment recommendations, and next steps. No. The psychologist provides an independent clinical opinion and completes the requested medical documentation. Veterans Affairs Canada makes the entitlement and benefit decision.

The comprehensive report can be provided directly to you, subject to the referral requirements, informed consent, and any applicable privacy or professional obligations. The report is intended to be useful not only for the VAC process, but also as a clear summary of the assessment findings, functional impacts, treatment recommendations, and next steps.
Who receives my information?
The intended recipients and limits of confidentiality are reviewed during informed consent. The questionnaire and report are released to VAC or the authorized referral source through the approved method. The comprehensive report may also be released directly to you and to other authorized recipients when permitted by consent, law, and professional obligations.
